Forms
To save some time during your first visit, we ask that you please download and complete the New Patient Packet.
There are three ways to complete the forms:
- Fill out the forms directly on your computer, then save and email them to us at: forms@eardoctor.org
- Print out your forms, complete them by hand, and bring them to your appointment
- Arrive 15 minutes early to your scheduled appointment, and our staff will provide you with the forms to complete when you check in

What to Expect at Your First Visit
During your initial visit, we will gather important background information about your medical history, and you’ll have the opportunity to get to know your medical doctor or audiologist. Depending on the complexity of your evaluation, the appointment may take anywhere from one to two hours. Please allow sufficient time for us to complete the examination and provide you with the best possible care.
