If you’re experiencing tinnitus (ringing/roaring in the ear), we kindly ask that you also fill out the additional forms listed below to provide us with more information to better serve you.

To save time during your upcoming visit and to help us refresh your chart, please download and complete the Annual Update Form before your appointment.

There are three ways to complete the forms:

  1. Fill out the forms directly on your computer, then save and email them to us at: forms@eardoctor.org
  2. Print out your forms, complete them by hand, and bring them to your appointment
  3. Arrive 15 minutes early to your scheduled appointment, and our staff will provide you with the forms to complete when you check in

VNG Testing Instructions & Forms

If you have dizziness, vertigo, or poor balance, you may have been asked to schedule a follow-up visit for a VNG test. Videonystagmography (VNG) is used clinically to evaluate patients with dizziness, vertigo, or balance dysfunction. You will find specific instructions that must be followed prior to your VNG test appointment, along with a questionnaire that your provider will need you to complete in the VNG Packet listed below.
